Though the hands-on work of implementing person-centered approaches often happens far from conference rooms, there is no denying that much of the planning, coordination and evaluation of these efforts occurs during meetings — meetings of your person-centered care committee, of task forces and action teams. How do you know if these meetings are effective and constructive? By asking participants. This tool can be used to prompt participants to rate the quality of these meetings based on their ability to prepare, to participate and to progress on action items.
